Here is the full list of things you will need to fill out your SSDI application accurately:<\/p>\n

  • if you were born outside the U.S.\n

    • give SSA<\/strong> the name of your country of origin<\/li>\n
    • give the Agency<\/strong> your Permanent Resident Card number<\/strong><\/li>\n<\/ul>\n<\/li>\n
    • Name of your current spouse (if any)<\/li>\n
    • Name of your prior spouse (if any)<\/li>\n
    • starting and ending dates of marriage(s)<\/li>\n
    • (optional) Spouse’s Social Security Number<\/strong> and date of birth<\/li>\n
    • Give SSA the names of the children who:\n
        \n
      • had a disability before age 22<\/li>\n
      • are unmarried and under 18<\/li>\n
      • are 18-19 but still in secondary school full time<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n

        SSDI required info<\/h2>\n

        Since SSDI<\/strong> are monthly payment from the Social Security Administration, you will need to give SSA the bank routing number<\/strong> and account type and number.
